Driver Pre-Screen Form

Before you begin…

Please fill out the first page of the IRS form 8850 (Pre-Screening Notice and Certification Request for the Work Opportunity Credit). You don’t have to include your social security number or signature. The form is available at: https://www.irs.gov/pub/irs-pdf/f8850.pdf and can be filled online.

Save the first page as a PDF and send as an attachment to employment@kool-pak.com. (Click on the print icon and choose “Save as PDF” as destination on your browser).
